Operate and Improve a Process

Keep a process controlled, useful, measurable, and adaptable after design and implementation end.

Adopt · export.mdAdopt · export.jsonDraft reference — not a fitted standard

Profile

Purpose

Give an operating owner a repeatable way to run the process, respond to exceptions, interpret evidence, and make controlled improvements without eroding the mechanism.

Intended outcome

A healthy process whose outcomes, quality, flow, use, exceptions, and guardrails are visible—and whose material changes are tested, authorized, versioned, and connected.

Accountable
Business process owner
Operator
Process operator or manager
Use when
  • — A new or redesigned process has met operational-acceptance criteria.
  • — A process is active but lacks an owner, review cadence, measures, or change control.
  • — Recurring workarounds, complaints, delays, defects, or local variations are accumulating.
  • — Leaders need to decide whether to maintain, improve, redesign, automate, or retire a process.

Entry gate

  • An approved process version, accountable owner, operating owner, and scope exist.
  • Entry and exit gates, workflow, roles, interfaces, controls, data, systems, measures, and exception paths are accessible.
  • Baseline or initial operating expectations and review cadence are defined.

Exit gate

  • The process is transferred to a successor version, materially redesigned, retired, or replaced through authorized change.
  • Open work, exceptions, records, owners, controls, measures, and downstream effects are transferred or closed.
  • The disposition and rationale are preserved in the process history.

Flow · 6 controlled steps

  1. 01

    Establish operating control

Confirm that the process can be run and governed from one authoritative definition.

    • — Confirm the active version, scope, owners, gates, controls, measures, systems, interfaces, and support paths.
    • — Accept transferred defects, risks, exceptions, and improvement commitments.
    • — Set operating, review, escalation, communication, and record-retention cadences.
    • — Confirm access and readiness for every operating role.
    Responsible

    Output
    Process Control Record
    Decision
Can the owner operate the process within its approved controls and support model?
  2. 02

    Run and observe the process

Produce the intended work while creating proportionate evidence about how the mechanism behaves.

    • — Operate normal work, decisions, controls, handoffs, and exception paths.
    • — Capture only the event, outcome, quality, timing, adoption, exception, and guardrail evidence needed to manage the process.
    • — Make ownership and status visible to upstream and downstream participants.
    • — Protect data, dignity, access, and appropriate use of workforce-related measures.
    Responsible
    Process operator or manager
    Output
Process outcomes · Operating observations · Measure events
  3. 03

    Triage and resolve exceptions

Restore flow safely while preserving information needed to remove recurring causes.

    • — Classify severity, affected work, immediate risk, owner, and response time.
    • — Contain harm and restore service using the authorized exception or recovery path.
    • — Escalate when decision rights, safety, quality, privacy, regulatory, or business thresholds require it.
    • — Record disposition and distinguish one-time events from recurring patterns.
    • — Do not convert repeated workarounds into undocumented standard practice.
    Responsible
    Process operator or manager
    Output
Exception Record · Restored or contained work
    Decision
Can this exception be resolved within current authority and process design?
  4. 04

    Review process performance

Interpret the measure set as a system and decide where attention is warranted.

    • — Review outcome, quality, cycle time, adoption, exceptions, and guardrails together.
    • — Compare with baseline, expected range, context, and material changes in demand or environment.
    • — Seek patterns, interfaces, common causes, unintended effects, and uncertainty rather than ranking individuals.
    • — Review participant and customer feedback alongside system evidence.
    • — Choose maintain, investigate, correct, improve, redesign, or retire.
    Responsible
    Business process owner
    Output
    Process Review Decision
    Decision
What response does the combined evidence justify?
  5. 05

    Design and authorize improvement

Change the smallest appropriate part of the mechanism without creating hidden inconsistency.

    • — State the observed problem, evidence, suspected cause, intended effect, and uncertainty.
    • — Generate options and assess effects on connected processes, roles, data, systems, controls, measures, users, and prior variants.
    • — Choose a proportionate correction, experiment, minor change, material redesign, or system implementation.
    • — Define validation, guardrails, rollback, authority, and version impact before action.
    • — Notify affected owners and obtain required approval.
    Responsible
    Process architect or improvement lead
    Output
    Improvement Proposal · Change Authorization
    Decision
Is the proposed response proportionate, testable, authorized, and safe for connected processes?
  6. 06

    Implement, verify, and learn

Close the feedback loop through controlled action and versioned learning.

    • — Execute the authorized correction, test, redesign, system change, or retirement path.
    • — Monitor intended effects and guardrails against baseline and stated assumptions.
    • — Adopt, revise, rollback, or reject the change based on evidence and authority.
    • — Publish the new version or operating clarification and update connected owners and materials.
    • — Record what was learned, including null or adverse results, for future standards and variants.
    Responsible
    Process architect or improvement lead
    Output
Controlled process update · Improvement Record · Updated connection notices
    Decision
What does the observed evidence support?
